ABOUT THE CENTER FOR WORK INJURIES

Since 1994, the Center for Orthopaedics & Sports Medicine has provided a full range of orthopaedic care to the greater Indiana, PA community. As surgeons and team physicians for many local high school and university sports teams, COSM has served the area’s athletes for over 15 years.

Our experience in sports medicine and returning the injured athlete to the field can be effectively applied to workers’ compensation conditions for injured employees. COSM’s new Center for Work Injuries is devoted to providing expeditious care to individuals suffering from work-related injuries. Our goal is to help the injured worker return to work in the shortest possible time through coordinated care and efficient communication.

POLICIES & PROCEDURES

Information about Patient Appointments

COSM completes a Workers’ Compensation Visit Information Form at each office visit. This form is given to the patient and automatically included with the billing statement as required by the Pennsylvania Workers’ Compensation Act.

Under our current policy, COSM can also release the patient visit information to the employer, third party administrators and case managers upon written consent from the patient. Patient signature is required on the Patient Release for Workers’ Compensation Visit Information. COSM uses only its forms and does not complete return-to-work or medical status forms submitted by employers.

For Case Managers

Case managers are permitted attend patient appointments under current policy and upon written consent from the patient on the Case Manager/Nurse Reviewer Appointment Authorization Form. The fee to attend a patient appointment (scheduled or unscheduled) is $100.00.

Case managers may call in advance to schedule their attendance at a patient appointment. A COSM scheduler will inform the case manager that they should contact the patient to advise them that they will be attending the office visit and that their consent will be needed. The patient should also be advised that the case manager will be in the presence of both the patient and the medical provider during their appointment. COSM medical providers do not see case managers separate from the patient visit.

If a case manager arrives unscheduled and wishes to attend a patient visit, a COSM scheduler will review the policy and procedures and advise that the patient that they will need to sign a consent form before the case manager can attend the patient appointment.

Additional notes about the patient, other than the Workers’ Compensation Visit Form, will not be released without patient consent and prepayment. COSM medical providers will not complete additional forms.

As a convenience, a Workers’ Compensation Billing Contact Information Form is provided so that the most current contact information is on file for the patient, employer, insurance carrier and case manager. This will help ensure efficient communications of visit information.

Release of Duplicate Records

Additional notes about the patient, other than the Workers’ Compensation Visit Form, will not be released without patient consent and prepayment. COSM medical providers will not complete additional forms.
Provider Panels

COSM accepts placement on workers’ compensation provider panels. COSM accepts only the current Pennsylvania State Workers’ Compensation fee schedule for payment of services. We do not participate in discounted provider networks.

ABOUT CWI’S DIRECTOR

Alan Keefe, MPA-C joined the Center for Orthopaedics and Sports Medicine in September 1999. He earned a Bachelor’s degree in Health Sciences and a Masters of Physician Assistant from Duquesne University in Pittsburgh, Pennsylvania.

Keefe is a certified physician assistant through the National Commission on Certification of Physician Assistants (NCCPA). He was recertified in 2011. Keefe is also certified by the Pennsylvania State Board of Medicine and the Pennsylvania State Board of Osteopathic Medical Examiners.

Keefe is a member of the American Academy of Physician Assistants, the Pennsylvania Society of Physician Assistants, the American Academy of Orthopedic Surgeons, and Physician Assistants in Orthopedic Surgery.
